"Extra-cranial proximal pica aneurysm - a rare and surreptious cause of posterior fossa sah: case report and review of literature".

Extra-cranial posterior inferior cerebellar artery (PICA) aneurysms are rare with only 22 cases been reported so far. Intra-dural type of extra-cranial PICA aneurysm is even rarer with few case reports available. We report a previously unreported type of proximal PICA aneurysm in which the PICA aneurysm had intra-dural location at the C2 vertebral level.

Identification of metabolism-associated molecular subtype in ovarian cancer.

Ovarian cancer (OC) is the most lethal gynaecological cancer with genomic complexity and extensive heterogeneity. This study aimed to characterize the molecular features of OC based on the gene expression profile of 2752 previously characterized metabolism-relevant genes and provide new strategies to improve the clinical status of patients with OC. Finally, three molecular subtypes (C1, C2 and C3) were identified. The C2 subtype displayed the worst prognosis, upregulated immune-cell infiltration status and expression level of immune checkpoint genes, lower burden of copy number gains and losses and suboptimal response to targeted drug bevacizumab. The C1 subtype showed downregulated immune-cell infiltration status and expression level of immune checkpoint genes, the lowest incidence of BRCA mutation and optimal response to targeted drug bevacizumab. The C3 subtype had an intermediate immune status, the highest incidence of BRCA mutation and a secondary optimal response to bevacizumab. Gene signatures of C1 and C2 subtypes with an opposite expression level were mainly enriched in proteolysis and immune-related biological process. The C3 subtype was mainly enriched in the T cell-related biological process. The prognostic and immune status of subtypes were validated in the Gene Expression Omnibus (GEO) dataset, which was predicted with a 45-gene classifier. These findings might improve the understanding of the diversity and therapeutic strategies for OC.

Association between ezetimibe usage and hepatitis C RNA levels in uninfected kidney transplant recipients who received hepatitis C infected kidneys.

Kidney transplantation (KT) from hepatitis C virus infected(HCV+) donors to HCV negative recipients achieve excellent graft function but have relatively higher rates of post-KT co-infections presumably due to prolonged HCV viremia in transmission-and-treat approach. Ezetimibe acts as an antagonist of Niemann-Pick C1-Like 1 receptor required for HCV entry and theoretically can reduce HCV viremia. However, no data is available to examine the role of ezetimibe as a bridge therapy between KT surgery and direct acting antiviral(DAA) initiation. A retrospective cohort study including 70 HCV+ to HCV negative KT recipients from Methodist University Hospital and Vanderbilt University Medical Center was performed to determine the association between ezetimibe usage and HCV viremia. Twenty patients received ezetimibe daily while 50 patients didn't. Primary outcome of study was mean HCV RNA level at 1-2 weeks post-KT and before initiation of DAA. Median(IQR) viral load(VL) in log copies/ml was one log lower in ezetimibe group versus non-ezetimibe group [4.1 (3.7-5.3) vs. 5.1 (4.4-5.5), p = 0.01], and highest VL was also lower in ezetimibe group [4.2 (3.7-5.4) vs. 5.4 (4.7-5.9), p = 0.006]. We concluded that ezetimibe bridge therapy might be associated with reduction in HCV VL while waiting for DAA initiation in HCV+ to HCV negative KT recipients. This article is protected by copyright. New natural furfural derivatives from the leaves and stems of .

A new furfural derivative, pogoscafuran A (), a new natural product, HMF levulinate (), together with four known compounds (-) were isolated from an extract of the leaves and stems of (Blanco) Benth. Their structures were elucidated on the basis of extensive spectroscopic analyses and single-crystal X-ray crystallography. Compound was the first example of natural furfural derivative with a unique C5-C1' linkage between a molecule of furfural and 3-methyl-2-cyclopentenone moiety. The plausible biogenetic pathway for the new compound was proposed. All these isolated compounds were tested for their inhibitory effects on the nitric oxide (NO) production induced by lipopolysaccharide in RAW264.7 cells, and only compound exhibited weak inhibitory activity.
